Athletic Participation & Physical Form
Parent/guardian and student sign this form to acknowledge permission to participate in interscholastic / school sponsored athletics as well as agreement to abide by the Sun Prairie High School Rules and Regulations. This form is required for the first sport in which the student participates each year.
For each sport in which a student participates, a blue medical history card, concussion acknowledgment, and athletic fee are also turned in to the athletic department office.

Concussion Acknowledgement Form - SPASD
A concussion acknowlegement form must be competed for EACH season in which your son/daugher participates. This form must be signed by both the parent/guardian and the student-athlete.
